Right at Home seeks to substantially reduce the number of individuals and families losing their homes, while laying the groundwork for a federal homelessness prevention strategy.
Our ultimate goal is to prevent hundreds of thousands of Americans from losing their homes, and provide a critical proof point for funding of a national homelessness prevention strategy that our country urgently needs.

The Right at Home Targeted Prevention Model

Prioritize those most likely to become homeless through shared assessment that incorporates the highest risk factors

Rapid intervention of financial assistance and supportive services, including legal services

Shared data and outcomes fully integrated into Coordinated Entry System (or Continuum of Care or HMIS)

Equitable and accessible networked delivery through community partners

Public and private partnership to ensure funding at scale AND flexibility to do whatever it takes to prevent homelessness and stabilize
Reducing inflow
In communities across the county, more people are being pushed into homelessness than the local systems can house. The only way we can turn this tide is with the help of targeted prevention programs. Santa Clara County, for example, is getting closer to housing more people than are pushed into homelessness, in part thanks to its growing Homelessness Prevention System.
